2. Wait for communication from the school 

Needed Documents

  • Birth Certificate (required by state law)

  • Immunization Records (required by state law)

  • Proof of Residence (piece of mail with current address), used to verify you live in the district 

  • Former School Information

Q: I already have a student enrolled in Mitchell, do I still fill out this form? 

A: No, log into your parent portal and register them there. 

Q: How long will it take the school to contact me after I fill out the form? 

A: Depending on the time of year, 1-3 business days. 

Q: I struggle using a computer, can I do this in person?
